Mobile Case Note Entry Services

Agreement for Services Provided


JewelCode Corporation is making available a service called Mobile Case Notes ("MCN") which allows you to enter information via a secure Internet link. JewelCode uses an encrypted SSL security certificate to protect the information that you enter; however, you agree that JewelCode will not be held accountable for security breaches resulting from your use of the web entry forms and you will bear the responsibility of protecting your and your client's private information.

JewelCode provides methods for you to protect your information such as private codes to protect your company name, employee names and client names. JewelCode also uses secure FTP to store the data that you enter and provides an import tool that pulls the data directly from your secure FTP location. There is no option to have data e-mailed to you. JewelCode also automatically deletes your entries after 7 days of storage in your FTP folder. JewelCode will provide additional protection by creating a randomized URL that is specific to your company only. You agree that it is your duty to consider using name masking and other techniques to protect your client's private information while using this service. JewelCode will, under no circumstances be responsible for any data loss or privacy violations from using this service.

If you are using an iPad, you understand that you must have an Internet connection available to enter notes into a web page to use this service. You may use it with Wi-Fi but you have to have a current Wi-Fi connection to use this service.

This service is unlimited and is priced based on the number of users that will be using the service. If it is found that more users are using the service than you have contracted with JewelCode, there will be a $50 time and services charge for failure to communicate the change to JewelCode.

If your payment method expires, declines or fails in some way, you will be given immediate notice and you will have three business days to correct the issue. After three business days your account will be suspended until payment is renewed.

The contract is on a month-to-month basis. Month-to-month means that if you cancel there are no refunds of money paid but your subscription will be discontinued as of the next billing cycle. If you contact us on the day of your billing cycle then we are not able to stop those payments as they run automatically. As long as you keep the contract current and solve billing issues within three business days you will be locked in at your original price for the duration of the contract. If your billing fails and is not corrected or if you choose to discontinue the contract and later restart then your fee will be at the new fee on the price list, at time of restart.

Plan to allow at least 90 minutes for your web entered records to be available for import into CareComplete™. The cycle to push them out for import is set to 60 minutes so 90 minutes will allow for processing time on each side of the process.

Because JewelCode utilizes 3rd party platforms for this service there may be downtimes. It is our goal to know of planned maintenance and to let you know; however, it is beyond our control if one of our service providers is down or your Internet service is interrupted. There are no refunds for downtime or other delays in the service that are beyond the control of JewelCode.

The system being used is such that it should be fully operational on weekends and holidays; however, if there is an outage for some reason then there will not be support for it until the next business day if it is a weekend or holiday.

Limitation of Liability: Client understands that JewelCode is not liable for any loss, claim, injury, liability or damage of any kind from using the Service or its associated software and hardware. Client agrees that any claims again JewelCode will not exceed $75.00, or the cost of one month of Service, whichever is smaller. Client accepts that they are responsible for all content and output and they will review and accept their own entries.

I agree with the terms set forward in the agreement above, for the mobile case note entry service and acknowledge that I have read the full agreement.
